Cruciate Care Ltd ("Cruciate Care," "we," "us") operates this website and the US Clinical Advisory Panel outreach form. This policy explains what information we collect from visitors and form submissions, why we collect it, and how it's handled.
When you submit the Clinical Advisory Panel form, we collect:
- Full name and professional title
- Clinic or practice name
- Email address
- Primary specialty
- Any optional selections you make (e.g. requesting a sample pilot box, scheduling a founder call)
We do not collect payment information, government ID numbers, or patient health data through this form.
We use the information you submit to:
- Respond to your inquiry regarding clinical evaluation or advisory panel participation
- Coordinate sample pilot box shipments, if requested
- Schedule founder introduction calls, if requested
- Understand practitioner interest as we prepare for US market launch
We do not use this information for unrelated marketing, and we do not sell your information to third parties.
Form submissions are processed through Formspree, a third-party form-handling service, which transmits your submission to us by email. Formspree may process your data in accordance with its own privacy policy. We do not control Formspree's infrastructure but have selected it as a standard, reputable service for this purpose.
We retain submitted information for as long as reasonably necessary to respond to your inquiry and, if you engage with us further (e.g. as a pilot participant or advisory contact), for the duration of that relationship. You may request deletion at any time (see below).
Regardless of your location, you may contact us to:
- Request a copy of the information we hold about you
- Request correction of inaccurate information
- Request deletion of your information
- Withdraw consent for further contact

If you are a California resident, you have rights under the California Consumer Privacy Act (CCPA), including the right to know what personal information is collected and the right to request deletion. If you are located in a US state with a comprehensive privacy law (e.g. Virginia, Colorado, Connecticut), you may have similar rights under that state's law. We honor these requests regardless of the state you're contacting us from.
This site does not currently use tracking or advertising cookies beyond what may be strictly necessary for basic site functionality.
We may update this policy as Cruciate Care's US launch progresses. Material changes will be reflected with an updated effective date above.
